To Meet Licensing and school Requirements, we need to have the following paperwork
for each student registered at Sunshine. The following forms are in PDF format.
You need Adobe reader to open these files. Please complete, sign, print and
bring the forms to school director before or on the day your child start Sunshine. Thank you.

- Identification and Emergency Information (LIC 700)
- Child’s Pre-Admission Health History Parent’s Report (LIC 702)
- Personal Rights (LIC 613A)
- Notification of Parents’ Rights (LIC 995)
- Consent for Medical Treatment Form (LIC 627)
- Playground and School Ground Release Form (Playground Release Form)
- Photography and Video Release Form (Media Release Form)
- After School Only: After School Policy (After School Policy)
- After School Only: Extra Curriculum Classes (Extra Curriculum Classes)
- Pre-School Only: Physician’s Report (LIC 701) and Immunization Record (Please bring a copy of yellow card)
